Establishes a New York State Brain Donor Registry to maintain a listing of individuals who consent to donate their brains for medical research on developmental disabilities, including autism spectrum disorders and cerebral palsy.

Allows registration through driver's license applications, non-driver ID card applications, voter registration forms, a department website, or other methods approved by the Commissioner.

Restricts registry information access to entities formally approved by the Commissioner and limits use to identifying potential brain donors at or near time of death.

Requires the Commissioner to notify individuals who registered before the law's effective date to explain the difference between intent and consent registrations and provide opportunity to update their registration status.

Amends vehicle and traffic law to allow driver's licenses and non-driver identification cards to display "BRAIN DONOR" designation and provide space for brain donation indication.
