Requires all health insurance policies providing medical, major medical, or comprehensive coverage to include prosthetic device coverage equal to or above Medicare's standard

Coverage must include provision, repair, and replacement of prosthetic devices required to be worn or used by policyholders, plus assessment, evaluation, treatment, management, and follow-up care expenses

Prohibits denial of coverage based on cosmetic purposes or lack of functional defect or impairment

Defines "prosthetic device" to include artificial limbs and artificial eyes but excludes shoes and ordinary wearing apparel

Takes effect 90 days after enactment and applies to policies issued, renewed, modified, altered, or amended on or after the effective date
