Authorizes the school districts of Rochester and Buffalo to directly provide or contract for integrated school health services, including school-based and school-linked health services using telemedicine and other emerging technologies.

Allows boards of cooperative educational services to contract with Rochester and Buffalo school districts to provide all or part of their integrated health services.

Entitles Rochester and Buffalo school districts to receive the same state aid funding and reimbursement as county health departments for providing school health services, retroactive to July 1, 2004 for Rochester and the first day of the fiscal year when county funding ceased for Buffalo.

Exempts school districts from municipal public health plan requirements when applying for state aid, instead requiring applications be submitted according to procedures set by the Commissioner of Education.

Makes the act effective immediately and applies retroactively to health services rendered on or after July 1, 2004 for schools where county funding was not available during the 2004-2005 school year.
