Authorizes the Commissioner of Health to establish medical home multipayor programs that certify clinicians and clinics as medical homes eligible for enhanced payments serving Medicaid, Family Health Plus, Child Health Plus, and commercial insurance enrollees.

Defines eligible clinicians as physicians, nurse practitioners, midwives, and specialists acting within their lawful scope of practice in primary care, gynecological care, or specialty care for chronic conditions.

Requires the Commissioner to actively supervise primary care medical home collaboratives (comprising health care providers and payors) to establish practice standards, payment methodologies, and data collection requirements consistent with national accrediting organizations.

Prohibits clinics and clinicians participating in multipayor programs from receiving additional enhancements under the statewide medical home program for Medicaid, Family Health Plus, or Child Health Plus services.

Requires the Commissioner to submit federal waivers necessary to implement the programs and annually report to the Legislature on program operation, effectiveness, quality, cost, and outcomes; no new programs may be approved after April 1, 2016.
