Establishes the "New York State Organ Donor Awareness and Education Act" to increase organ and tissue donation registration through motor vehicle registration and driver's license applications.

Requires motor vehicle registration and driver's license applications to include space for applicants to register or decline registration in the New York State Donate Life Registry, with a separate signature and consent statement in bold capital letters.

Permits applicants to specify particular organs or tissues for donation; registering as a donor does not affect medical treatment and is legally binding unless the donor is under 18 (in which case the legal guardian makes the final decision).

Creates a voluntary two-dollar donation option on vehicle registration and driver's license applications; the first $150,000 collected goes to the Department of Motor Vehicles for initial program costs, then 5% of subsequent collections cover administrative costs, with remainder deposited in the "Life Pass It On" Trust Fund.

Requires the Department of Motor Vehicles to distribute informational materials about organ donation, the Donate Life Registry, and the trust fund at registration and license applications, and to make materials available at all DMV offices and on its website.

Effective November 1, 2011.
