Establishes an annual $1,000 annuity from the state treasury for Purple Heart medal recipients who are New York residents and apply to the State Director.

Entitlement terminates upon death or departure from New York state residency, but can be reinstated if the recipient returns and reapplies.

Effective date of annuity award is the day after the Purple Heart is awarded if application is received within one year; otherwise the effective date is when the application is received by the State Director.

Denies annuity payments to recipients incarcerated in federal, state, or local penal institutions for any part of the period beginning 61 days after imprisonment begins through release.

Requires the State Director to submit annual reports to the Governor and legislative finance committees by January 15 each year, and takes effect 180 days after becoming law.
