Amends Public Health Law to require the Department of Health to contract with community-based and not-for-profit organizations for outreach and education services to naturalized citizens and immigrants on available health care services and benefits.

Contracted services must identify health care services available in residents' communities and include eligibility requirements for Medical Assistance, Emergency Medical Assistance, Family Health Plus, and Child Health Insurance Plan programs.

Services must provide information on how to apply for these programs and be delivered in languages and formats appropriate for the community served.

Department of Health must promulgate necessary rules and regulations, provide all educational materials, and pay costs for distributing materials to implement the program.

Act takes effect 180 days after becoming law, with implementation actions authorized to begin immediately.
