Requires providers of services to submit summary reports to the Office of Developmental Disabilities when terminating any employee or volunteer for abuse or neglect of a client, including the worker's name, home address, and social security number within 30 days of termination.

Requires providers of mental health services to submit similar summary reports to the Office of Mental Health when terminating employees or volunteers for abuse or neglect within 30 days of termination or investigation completion.

Mandates that both offices maintain summary reports together with criminal history information and share reports with other providers requesting background checks on the same individual, unless the provider notifies the office the person is no longer subject to background check requirements.

Requires both offices to notify affected employees or volunteers of the summary report against them and provide them the right to obtain, review, and submit a written response to the report, with any response transmitted to providers along with the original report.

Provides civil and criminal liability protections for summary reports disclosed by the offices in the same manner as existing protections for criminal history information disclosures; takes effect January 1 following enactment with commissioners authorized to adopt rules immediately.
