Establishes the "Donate Life Registry" as New York's organ and tissue donor registry containing listings of all donors who have declared consent to make anatomical gifts.

Expands registration methods to include driver license/non-driver ID applications, registry website enrollment with electronic signatures, voter registration forms, and any other method identified by the commissioner.

Allows registry information access only to federally designated organ procurement organizations, eye and tissue banks licensed by the department, and other entities formally approved by the commissioner, solely for identifying potential donors at or near time of death.

Amends state technology law to clarify that electronic signatures under article three of the state technology law apply to the registration of making, amending, or revoking anatomical gifts.

Takes effect immediately upon enactment.
