Designates the Office for the Aging as a participating provider for consumer directed personal assistance services under Social Services Law Section 365-F for individuals aged 60 and older who meet specified eligibility criteria.

Requires the Office to develop service models providing fiscal, supportive, and intermediary services including case management, peer counseling, employer skills training, attendant registries, and emergency backup services.

Authorizes the Office or contracted agencies to serve as employer of record while allowing eligible individuals to recruit, train, supervise, and discharge their own attendants, with the agency acting as fiscal agent.

Mandates the Office file a federal Section 1115 waiver application within 120 days to implement the program based on principles that eligible individuals are service experts, different services warrant different professional involvement levels, and choice and control should be available in all service delivery environments.

Grants the Office authority to waive conflicting provisions of Social Services Law and empowers it to provide services statewide without state agency audit limitations, except for oversight by the State Comptroller.
