Establishes the New York State Community Initiatives Project within the State Office for the Aging to coordinate services for seniors across multiple state agencies in up to 10 project areas per year, with at least one-third located in rural areas.

Requires each approved project to develop an action plan through a Community Initiatives Advisory Council assessing senior needs and describing actions and resources necessary to meet those needs immediately and over a five-year period.

Creates a State Community Initiatives Oversight Committee composed of representatives from 17 state agencies with authority to approve projects, provide technical assistance, waive regulatory barriers, and identify statutory impediments to implementation.

Authorizes the Comptroller to transfer funds between state agency appropriations as needed to fulfill unified contracts between the Office for the Aging and approved project applicants, with quarterly reporting to legislative committees.

Requires comprehensive evaluation and annual reporting to the Governor and Legislature on project effectiveness and feasibility of replication, with the program set to expire on April 1, 2017.
