Amends Public Health Law section 3008 to create an exemption from regional and state Emergency Medical Services Council review for cities with a population of 14,700 or 62,235 (according to the 2010 federal census) or fire districts acting on their behalf.

Allows these specified cities and fire districts to apply directly to the Commissioner for permanent certification of municipal advanced life support or ambulance services after a two-year temporary period without requiring a determination of public need from regional or state councils.

Applications shall be approved by the Commissioner unless the service has failed to meet appropriate training, staffing, and equipment standards.

Takes effect immediately and applies to municipalities or fire districts with existing temporary determinations of public need or pending applications in administrative or judicial appeals at the time the law becomes effective.
