Amends the Social Services Law to require the Commissioner of Health to develop a standard prior authorization request form for managed care providers to use when requesting utilization review determinations for prescription drug coverage

Mandates that all managed care providers utilize the standard form or forms developed by the Commissioner of Health for prescription drug prior authorization requests

Requires managed care providers to make the standard prior authorization form available to prescribing providers in both paper and electronic formats

Takes effect immediately upon enactment
