Adds new section 1352-d to New York Public Health Law requiring restaurants to meet nutritional standards when offering incentive items (toys, games, trading cards, admission tickets, or digital access to such items) with food or beverage purchases.

Directs the Commissioner of Health to establish nutritional standards covering calories, sodium, percentage of calories from fat, sugars and caloric sweeteners, and fruits, vegetables, and grains for meals, food items, and beverages offered with incentive items.

Applies to all eating and beverage establishments including coffee shops, cafeterias, fast food establishments, diners, and similar venues that serve food or beverages to the public.

Establishes civil penalties for violations: up to $500 for first violation, up to $1,000 for second violation, and up to $2,500 for third or subsequent violations.

Takes effect 120 days after becoming law.
