Mandates creation of a supplemental notice form listing all required notifications to applicants, with font size of at least 16 points for all notifications.

Requires the supplemental notice form to include check-off boxes for applicants to confirm receipt and understanding of each notification, plus a signature line for overall confirmation.

Local social services districts must provide all public assistance applicants with the supplemental notification form.

Office of Temporary and Disability Assistance must review and update the supplemental form at least every six months to add any new notifications required by law, rule, or regulation; effective date is 180 days after enactment.
