Allows adopted persons age 18 and older to petition the court for a non-certified copy of their original long-form birth certificate or identifying information about their birth parents.

Requires the Department of Health to make reasonable good-faith efforts within 120 days to notify birth parents that an adopted person has requested their information, and allows birth parents to file written confirmation of consent or request continued confidentiality.

If a birth parent consents to release of identifying information, the court shall order release of the full birth certificate; if a birth parent requests confidentiality, the birth certificate shall be released with identifying information redacted.

If a birth parent cannot be located or does not respond after good-faith efforts, the court may release unredacted information unless it determines such release would be clearly detrimental to the welfare of birth or adoptive parents.

Establishes that birth parents who consent to release of identifying information may complete a contact preference form and medical history form to accompany the released information, and directs the Department of Health to conduct a public information campaign about the adoption information registry; effective January 15, 2015.
