Establishes a "behavioral health and long-term care plan" to be developed and monitored jointly by the commissioners of mental health, health, alcoholism and substance abuse services, and the director of the office for the aging.

Requires the plan to coordinate provider services and interagency referrals for persons with mental illness and/or chemical dependence who need long-term care, excluding dementia cases.

Directs the plan to include recommendations for integrating federal, state, and local funding sources; facilitating research on best practices; increasing alternatives to nursing homes; and supporting family caregivers.

Mandates the plan include specific implementation actions, number of people to be served, measurable phases, and a time schedule for achievement.

Requires the commissioner of mental health to submit a report to the governor and legislative leaders by October 1, 2016, and annually thereafter through 2024 on plan implementation, persons served, and measurable outcomes.
