Establishes the Child Support Military Modification Program (CSMM) to allow temporary modification of child support obligations for New York National Guard and Reserves members called to active duty for more than 30 continuous days in overseas or continental United States military operations.

Requires the Commissioner of the Office of Temporary and Disability Assistance to create an application form for eligible members, available on the agency's website and at military mobilization centers, that authorizes access to military income information.

Directs child support enforcement units to file a motion in court to temporarily modify child support payments proportionally based on the difference between the member's military pay and civilian pay if income varies significantly.

Temporary modifications apply from the date of activation through the duration of active duty deployment plus 60 days following deactivation, after which the original court-ordered child support obligation returns to full force.

Excludes service during training exercises and mobilization for disaster relief projects from program eligibility.
