Requires health insurance policies providing medical or major medical coverage to include coverage for prosthetic devices equal to or above Medicare's standard of coverage

Mandates coverage for provision, repair, and replacement of prosthetic devices, as well as assessment, evaluation, treatment, management, and follow-up care expenses

Prohibits insurers from denying coverage if a physician determines the prosthetic device is the most appropriate model meeting the patient's medical needs for daily living or job-related activities

Defines "prosthetic device" to include artificial limbs but excludes shoes and ordinary wearing apparel, whether or not specially constructed

Applies requirements to health insurance policies issued by Blue Cross Blue Shield plans, health maintenance organizations, and other insurers; takes effect May 1, 2015, with amendments effective April 1, 2017
