Prohibits the New York City Department from selling or distributing patient protected health information without express written authorization from patients.

Requires that patient consent be provided in a separate, stand-alone document and limited to the specific use or transaction for which consent is given.

Extends the same prohibitions to emergency service providers statewide under the General Municipal Law.

Exempts patient information transmitted to insurance carriers that is necessary to process insurance claims from the consent requirement.

Takes effect immediately upon enactment.
