Creates a new database of programs shown to be successful in supporting and treating elderly persons (age 60+) with dementia or Alzheimer's disease, to be developed and maintained by the Office in conjunction with the Department of Health.

Requires the Director and Commissioner of Health to develop criteria for determining which programs are considered successful and eligible for database inclusion, with option to consult public, governmental, non-profit, and for-profit agencies.

Makes the database publicly accessible on the Office's website with information including contact details, program name, brief description, and geographic service area.

Mandates the Office update the database at least once annually in conjunction with the Department of Health.

Takes effect one year after becoming law, with authorization to adopt necessary rules and regulations for implementation.
