Establishes the New York Access to Home for Heroes Program within the Private Housing Finance Law to provide grants for adapting or retrofitting homes for disabled veterans.

Defines eligible applicants as cities, towns, villages, or not-for-profit corporations with at least one year of existence and substantial experience adapting homes for persons with disabilities.

Defines eligible disabled veterans as state residents with honorable or general discharge who are certified by the U.S. Department of Veterans Affairs or Department of Defense as entitled to disability payments for service-related injuries.

Limits individual contracts to a maximum of $500,000 with a three-year completion period and allows up to 7.5 percent of contract amounts for administrative costs, while requiring no more than 50 percent of annual funds be allocated to any single municipality.

Requires eligible applicants to submit plans addressing program feasibility, community impact, budget, completion schedule, and affirmative action and minority business participation before receiving funds.
