Requires mental health discharge planning for inmates who received mental health treatment within three years of their anticipated release from state correctional facilities.

Mandates appointment with a community mental health professional capable of prescribing medications following discharge, when necessary.

Requires provision of sufficient mental health medications and prescriptions to cover the period between discharge and when community care begins.

Allows inmates who refused mental health treatment to also receive discharge planning and community mental health professional appointments if necessary.

Expands authority under mental hygiene law to include Department of Corrections and Community Supervision designees responsible for community supervision in regions where released individuals are placed.
