Creates a heroin and opioid addiction wraparound services demonstration program under the Department of Mental Hygiene in consultation with the Department of Health

Provides wraparound services to adolescent and adult patients during treatment and for up to nine months after treatment completion

Wraparound services include case management (education, legal, financial, social, family, and childcare services), peer support groups, employment support, and transportation assistance

Requires the Commissioner to submit a written evaluation to the Governor and legislative leaders within two years assessing program effectiveness and recommending continuation or expansion

Expires and is repealed three years after effective date
