Replaces witness requirement for health care proxies with notarization: health care proxy must be sworn before a notary public instead of signed before two adult witnesses.

Prohibits health care agents from making decisions to end a person's life without written opinions from two independent medical doctors (not the attending physician) agreeing with the decision.

Requires death certificates to indicate when a death results from a health care agent's decision to withdraw nutrition or hydration.

Takes effect immediately upon enactment.
