Creates a new voter registration method allowing qualified persons to register to vote simultaneously with applications for hunting and/or fishing licenses through the Department of Environmental Conservation.

Requires the Department of Environmental Conservation to design integrated forms with the approval of the State Board of Elections, with all design costs borne by the Department of Environmental Conservation.

Mandates voter registration forms include eligibility requirements, attestation under penalty of perjury, opt-in checkboxes, and notice of rights including confidentiality protections and procedures for filing complaints regarding interference with registration rights.

Requires the Department of Environmental conservation to transmit completed registration applications to the appropriate Board of Elections within ten days of receipt, or by the twentieth day before an election if received between the thirtieth and twenty-fifth day before an election.

Establishes that registration information and declinations are confidential and can only be disclosed for voter registration purposes, with violations constituting a breach of privacy under Public Officers Law; provides the State Board of Elections authority to adopt implementing rules and designates a voter registration coordinator within the Department of Environmental Conservation.
