Creates Section 2805-w of Public Health Law establishing a hospital-home care-physician collaboration program to support voluntary initiatives improving patient care access, health outcomes, cost-effectiveness, and community population health.

Authorizes the Commissioner to provide financing through grants, medical assistance rate adjustments, premium payments, federal waivers, and state appropriations, as well as regulatory waivers to support collaborative initiatives.

Permits collaborative initiatives including transitions of care programs, clinical pathways, telehealth/telemedicine services, physician house calls, readmission prevention models, health home development, and bundled payment demonstrations for post-acute care.

Allows initiatives for recruitment, training, and retention of hospital and home care staff and physicians in areas of demonstrated need, including cross-training opportunities and salary/benefit enhancements.

Requires participating hospitals and home care agencies to report to the Commissioner on patient, service, and cost experiences, with reports compiled and made available on the Department's website.
