Permits former foster care youth to file motions to return to custody of the social services district or authorized agency that previously held their placement, with court jurisdiction extending to hear such motions for up to 24 months after discharge.

Requires youth reentering foster care to demonstrate no reasonable alternative exists, consent to educational or vocational program enrollment (unless court finds it unnecessary), and show reentry is in their best interests.

Authorizes courts to grant reentry motions if a social services district's refusal to consent is found unreasonable, and limits youth to two successful reentry petitions with stricter standards for subsequent motions.

Mandates that social services officials provide notice to youth aging out of foster care of their right to apply for reentry within 24 months of final discharge or their twenty-first birthday, whichever is earlier.
