Removes the July 1, 1996 age cutoff date that previously limited eligibility for transitional care services, allowing individuals who become twenty-one after that date to qualify.

Requires the Office to provide written notice by certified mail at least forty-five days before changing a person's placement, including description of the proposed placement, appeal procedures, and contact information.

Establishes a formal administrative appeal process with a hearing before the commissioner or designee, requiring a written determination within thirty days of the hearing, with final determinations subject to Article 78 judicial review.

Expands notification requirements to include "another individual who has been involved in the care of the person" in addition to the person and their guardian when applicable.

Requires the Office to schedule a hearing within five days of receiving a written appeal request and provide at least ten days' notice to the objecting party.
