Creates a new "Access to Home for Heroes Program" under the Private Housing Finance Law to provide financial assistance to disabled veterans for home adaptations and retrofitting

Eligible applicants include cities, towns, villages, or not-for-profit corporations with at least one year of existence and substantial experience adapting homes for persons with disabilities

Defines eligible disabled veterans as state residents with honorable or general discharge who are certified by the U.S. Department of Veterans Affairs or Department of Defense as entitled to disability payments for service-related injuries

Authorizes the Housing Trust Fund Corporation to award grants up to $500,000 per contract with no more than 50 percent of annual funds allocated to any single municipality, with a maximum three-year program completion period

Requires eligible applicants to submit plans detailing program feasibility, community impact, budget, completion schedule, and affirmative action/minority business participation goals, and permits up to 7.5 percent of contract funds for administrative costs
