Requires state correctional facilities to provide mental health discharge planning for inmates who received, refused, or failed to comply with mental health treatment within three years of their anticipated release date.

Mandates appointment with a community mental health professional who can prescribe medications and provision of sufficient medications and prescriptions to bridge the period between discharge and assumption of care by the community provider.

Requires clinical assessment to determine if inmates meet criteria for involuntary admission or assisted outpatient treatment under Mental Hygiene Law, with the Department executing necessary applications and petitions if warranted.

Authorizes the Department of Corrections and Community Supervision designee responsible for community supervision in the release region to petition for involuntary mental health admission or assisted outpatient treatment.

Takes effect 60 days after becoming law.
