Creates a new Penal Law section 178.26 making it a Class A misdemeanor to willfully obtain or attempt to obtain controlled substances or prescriptions through fraud, deceit, misrepresentation, false statements, false identity, or forged documents.

Establishes presumptive evidence that possession of a false or forged controlled substance prescription (by non-pharmacists) or blank official New York State prescription forms (by unauthorized persons) indicates intent to illegally obtain controlled substances.

Prohibits individuals from intentionally concealing or failing to disclose that they are receiving controlled substances or prescriptions from multiple practitioners during concurrent treatment.

Applies provisions from Public Health Law Section 3396 to this new offense.

Takes effect immediately upon enactment.
