Requires policyholders to provide written notice to certificate holders and labor organizations at least 90 days before substituting an insurance policy that alters, restricts, or terminates contraceptive coverage, with copies to the Commissioner and Departments of Law and Financial Services.

Requires employers offering group health insurance to notify all job applicants whether their policy includes contraceptive coverage, specifying any excluded drugs or devices, either on employment applications or separate written notice forms and on employer websites.

Establishes penalties up to $5,000 per violation for employer non-compliance, with corporate officers (president, secretary, treasurer) held personally liable.

Takes effect 90 days after becoming law.
