Amends Section 4132 of the Public Health Law to add the new parental designation option

Requires the Department of Health Commissioner to promulgate rules and regulations for implementation immediately upon passage

Takes effect on January 1 of the year following enactment

Known as the "Modern Family Birth Certificate Act"
