Amends Public Health Law Section 4012-b to revise the definition of palliative care that hospices may provide to patients and families.

Changes the eligible patient population from "advanced and progressive disease" to "life-limiting conditions and illnesses" to align with definitions used elsewhere in public health law.

Redefines "palliative care" as active, interdisciplinary care focusing on relief of physical and psychosocial symptoms and spiritual needs, with the goal of achieving the best quality of life for patients and families.

Permits hospices to provide palliative care either independently or under contract with certified home health agencies, long-term home health care programs, or licensed home care services agencies.

Takes effect immediately upon enactment.
