Establishes Article 28 of the Private Housing Finance Law creating the "New York Access to Home for Heroes Program" to provide financial assistance for home adaptations for disabled veterans.

Defines eligible applicants as cities, towns, villages, or nonprofit corporations with one or more years of existence and substantial experience adapting homes for persons with disabilities.

Authorizes the Housing Trust Fund Corporation to enter into contracts with eligible applicants for grants not exceeding $500,000 per contract, with programs to be completed within three years (extendable by up to two additional one-year periods for good cause).

Limits any single municipality to receive no more than 50 percent of total funds awarded in any fiscal year and requires geographic distribution efforts for statewide impact.

Permits eligible applicants to spend up to 7.5 percent of contract amounts for administrative costs and requires submission of plans including program feasibility, budget, completion schedule, and minority business participation.
