Establishes "Developmental Disability Individual Support and Care Coordination Organization" (DISCO) as a new category of managed care organization authorized by certificate from the Commissioner of Health and Commissioner of Developmental Disabilities under Public Health Law § 4403-G.

Exempts DISCOs from standard managed care requirements including primary care practitioner selection rules, early periodic screening and treatment services, prenatal care services, and chemical dependence treatment access requirements unless specifically authorized.

Allows participants to change to or between DISCOs at any time without cause, unlike standard managed care providers which have annual change restrictions.

Permits DISCOs to limit participants to one managed care plan if federal approval is obtained and fewer than two DISCO plans operate in the participant's county of residence.

Repeals Mental Hygiene Law § 5.07(c) and § 13.06, and revises appointment procedures for developmental disabilities regional office directors and state operations office boards of visitors to remove Senate consent requirements.
