Requires the Commissioner of Mental Hygiene to ensure continuity of care for individuals with developmental disabilities when state-operated services are closed, consolidated, merged, or otherwise diminished

Mandates that affected individuals be offered the option to transition to state-operated services within their current Developmentally Disabled Service Offices region, and remain in their current facility until requested services become available

Requires the Commissioner to document all offers of state-operated opportunities and maintain records of services offered to individuals with developmental disabilities

Directs the Commissioner to track unmet service needs when requested services are unavailable and develop a plan to address those needs in the following fiscal year

Requires submission of the unmet needs plan to the Temporary President of the Senate and Speaker of the Assembly by December 31st each year
