Authorizes New York residents to request distinctive license plates bearing the phrase "Cystic Fibrosis Foundation" to support cystic fibrosis awareness and research.

Imposes an additional annual service charge of $25 on each Cystic Fibrosis Foundation license plate, in addition to regular registration fees.

Establishes the "cystic fibrosis research and education fund" to receive all revenues from the $25 annual service charges and other appropriated funds or grants.

Requires the Department of Health to approve all fund expenditures, which must be limited to cystic fibrosis research and education projects, and mandates annual reporting to legislative leadership on fund usage.

Requires a $6,000 surety bond from the Department of Motor Vehicles before plate production, unless at least 200 orders are received in advance; requires at least 200 plates to be sold within two years or the bond may be recovered against.
