Amends Social Services Law to require the Department of Health and Mental Hygiene in cities with populations of one million or more to issue a performance summary card to each permitted child care service.

Performance summary card shall summarize health and safety inspection results and compliance with applicable laws, including recent inspection violations and suspensions.

Card may include additional information such as facility capacity, length of operation, and comparisons to other child care services in the same social services district.

Child care services must conspicuously post their most recently issued performance summary card at or near the entrance in accordance with department rules.

Department must update and reissue the performance summary card at least once every twelve months; the act takes effect one year after enactment.
