Requires Poplawski's reinstatement request to be notarized and submitted in writing to the Department of Taxation and Finance within 90 days of enactment, with the election being irrevocable.

Mandates the Department of Taxation and Finance provide written notice to the New York state and local employees' retirement system at least 30 days after receiving Poplawski's reinstatement request.

Authorizes the State of New York to make required employer contributions for Poplawski's optional retirement program membership, with the State University of New York administering the account and potential reimbursement from the retirement system.
