Requires group and blanket accident and health insurance policies to cover diagnostic and treatment procedures for infertility, including prescription drugs and in vitro fertilization.

Defines infertility as either a diagnosis by a licensed physician or failure to establish pregnancy after twelve months of unprotected sexual intercourse.

Mandates coverage of medically necessary fertility preservation services when medical treatment may cause iatrogenic infertility (fertility impairment from surgery, radiation, chemotherapy, or other medical treatment).

Prohibits insurers from discriminating in coverage determinations based on age, sex, sexual orientation, marital status, expected life length, disability, or medical dependency.

Exempts small group market insurance policies from these coverage requirements and removes prior age restrictions (21-44 years) on infertility coverage.
