Eliminates annual deductibles and coinsurance for breast cancer screening and diagnostic imaging (including mammograms, ultrasounds, and MRI) under health insurance policies and contracts, effective January 1, 2017.

Requires certified mammography facilities to provide extended hours for screening services on at least two days per week for at least two hours each day, including early morning (7-9 AM), evening (5-7 PM), or weekend hours (9 AM-5 PM Saturday/Sunday).

Allows facilities to request waivers from extended hours requirements if they lack sufficient staff, are discontinuing services, or face other hardships deemed appropriate by the Department of Health.

Limits the no-deductible and no-coinsurance coverage requirement to participating providers in an insurer's network, unless no in-network provider with appropriate training and experience is available.

Eliminates the exemption of cities with populations over one million from civil service leave provisions for breast cancer screenings, effective 90 days after enactment.
