Allows employers to apply for exemption from workers' compensation coverage for employees who are members of recognized religious sects opposed to accepting insurance benefits on grounds of conscience.

Requires the application to include a written waiver by the employee of all workers' compensation benefits and an affidavit stating the employee's religious opposition to any public or private insurance for death, disability, retirement, or medical costs.

Grants exemptions if the Department finds the employee is a member of a sect with established tenets against insurance and that the sect has provided for dependent members for a substantial number of years in a reasonable manner.

Permits a minor employee's guardian to execute the required waiver and affidavit on the minor's behalf.

Exemptions remain valid for all future years unless the employee or religious sect ceases to meet the exemption requirements.
