Requires the Division of Veterans' Affairs to maintain and annually update a fact sheet on its webpage titled "Information for Veterans concerning Health Care Options" containing contact information for all veterans integrated service networks in New York and U.S. Veterans Health Administration facilities.

Amends hospital admission procedures to require staff to inquire whether admitted patients have served in the United States armed forces instead of using the previous definition of "veteran."

Mandates that hospital staff notify veteran admittees of possible services available at Veterans Administration hospitals and arrange transfers upon request, provided the patient's condition has stabilized and transfer can be safely accomplished.

Requires hospitals to provide all admittees who have served in the armed forces with a copy of the veterans' health care options fact sheet before discharge or transfer.

Section 1 takes effect 90 days after enactment; Section 2 takes effect 180 days after enactment.
