Authorizes the commissioner to release certain records of deceased individuals who resided in OPWDD-operated or certified facilities to family members upon written request.

Requires the commissioner to promulgate regulations specifying which information shall be released, including name, dates of admission/discharge/birth/death, cause of death, treatment, services provided, employment, and residential history.

Prohibits record release if the deceased individual or a qualified person (for those lacking capacity) previously objected to release in writing or through documentation in the individual's services plan, unless the objection was withdrawn.

Amends sections 13.09 and 33.13 of the Mental Hygiene Law to establish these record-release provisions notwithstanding existing confidentiality restrictions in sections 33.13 and 33.16.
