Amends the Medicaid nursing home capital reimbursement cap authorization to allow the Department of Health to adjust caps for a residential health care facility construction project by the Jewish Home of Rochester on a demonstration basis without requiring a request for proposals.

Expands existing demonstration authority that previously covered only the Capital Region Rehabilitation Center (Eddy Cohoes Rehabilitation Center) to now include the Jewish Home of Rochester.

Requires eligible facilities to report annually to the department on patient numbers, types of services provided, and outcome and financial data demonstrating the efficacy of the residential model.

Takes effect immediately upon enactment.
