Establishes a 90-day statewide synthetic cannabinoid surrender program allowing individuals to anonymously surrender products containing synthetic cannabinoids to the Department of Health at multiple locations.

Prohibits any person, firm, or entity from selling, offering for sale, distributing, or giving away synthetic cannabinoids for any purpose, with first violations subject to $2,000 civil penalty and subsequent violations within 5 years subject to $5,000 penalty.

Imposes a 5-year license prohibition for sellers of cigarettes, tobacco, lottery tickets, and alcoholic beverages upon third or subsequent synthetic cannabinoid sale violations within 5 years.

Authorizes municipalities to enact or enforce local laws governing licenses for businesses found to have unlawfully manufactured, distributed, or sold synthetic cannabinoids or synthetic phenethylamines.

Takes effect 180 days after enactment, with implementation actions authorized to begin immediately upon passage.
