Requires the Commissioner of Health to review and approve any closure of a nursing home or residential health care facility before it can proceed.

Commissioner must submit a written report within 30 days of a closure application in cities with populations of one million or more to community boards, city council members, state senators, assembly members, congressional representatives, borough presidents, city planning commissions, and city health departments.

Report must assess the closure's impact on community access to nursing care, measures to mitigate harm, resident relocation plans, and the state's ability to assume financial responsibility or identify an alternate operator.

Commissioner must post the report on the department website and display it conspicuously in the affected facility so residents and families can review it.

Commissioner may only approve closure if reasonably determining that the community's and stakeholders' needs for nursing care and health services can be adequately met, with a minimum 30-day waiting period after community review and recommendations are submitted.
