Allows New York State Elks Association members to request a distinctive license plate bearing the words "Elks Association" upon proof of active membership.

Requires applicants to submit annual verification of membership to the Department of Motor Vehicles in the form and detail prescribed by the commissioner.

Charges an additional annual service fee of $25 for the distinctive plate, in addition to the regular registration fee.

Requires a $6,000 surety bond from a state-authorized surety company before plate design and production, unless the commissioner receives at least 200 pre-orders with fees (which are non-refundable) prior to production.

Bond requirement is waived if 200 or more plates are sold within two years of first becoming available; if fewer than 200 are sold, the state may recover proportionate amounts against the bond.
